Position Overview
We are seeking a compassionate and motivated Case Manager to join our Coordinated Specialty Care (CSC) program. This role is essential in supporting individuals as they navigate mental health services, community resources, and recovery-focused care.
The Case Manager works closely with individuals, families, and an interdisciplinary clinical team to provide person-centered support, facilitate access to resources, and assist with transitions from inpatient or higher levels of care. This position emphasizes cultural humility, recovery-oriented practice, and community-based engagement.
What You’ll Do

Support individuals in accessing benefits, financial assistance, housing options, and community resources

Assist individuals in building life skills, daily routines, and achieving meaningful recovery goals
-
Coordinate care and supports with families and community partners, ensuring cultural relevance and accessibility
-
Support transition planning for individuals entering or exiting higher levels of care
-
Provide education and consultation to individuals, families, and medical professionals
-
Collaborate with physicians, social workers, nurses, and clinical staff to support treatment planning
-
Complete accurate, timely documentation and maintain required records
-
Participate in program planning, goal setting, and quality improvement initiatives

Attend weekly interdisciplinary team meetings
-
Assist with data collection related to service utilization and compliance as needed
-
Arrange non-medical transportation (NMT) through Medi-Cal when applicable
-
Meet individuals in homes, schools, workplaces, and community settings as part of assertive case management

Perform other duties as assigned
What We’re Looking For
Required Qualifications
-
Bachelor’s degree from an accredited college or university, or equivalent experience
-
At least one (1) year of experience providing case management services
-
Valid California driver’s license and ability to be insured under organizational policy
-
Ability to complete background check, drug screening, and TB test
Preferred Qualifications
-
Three (3) years of progressively responsible case management experience
-
Two (2) years of experience working with individuals experiencing psychosis
-
Experience working with youth and families
Key Skills & Competencies
-
Strong interpersonal and communication skills
-
Ability to assess needs and collaborate on care planning
-
Knowledge of mental health systems, community resources, and California systems of care
-
Familiarity with case management models, transition planning, and crisis intervention
-
Understanding of federal and state healthcare compliance regulations
-
Commitment to confidentiality, cultural humility, and person-centered care
