Intake Coordinator
You are a detail-oriented and compassionate
professional who ensures a smooth patient transition into home health services.
You thrive in a fast-paced environment, communicate effectively with patients,
families, and healthcare providers, and maintain strict adherence to
regulations and organizational standards.
Location: Oakland, San Jose, Burlingame or Fairfield
Pay: $30-$35/hr
Hours: Mon–Fri 9:00 am–5:00 pm, with one weekend day every other weekend
What you will be doing:
- Receive,
review, and process referrals from hospitals, physicians, and other
healthcare providers. - Verify
patient eligibility, insurance benefits, and authorization requirements. - Collaborate
with clinical teams and case managers to schedule timely admissions. - Serve
as the primary contact for patients, families, and providers, providing
clear and compassionate guidance. - Maintain
accurate documentation in EHR systems, CRM, and logs while ensuring HIPAA
compliance. - Communicate
with internal teams and external partners to resolve barriers to care and
escalate issues as needed.
What you bring:
- Minimum
of 2 years of experience in referral intake, patient
registration/admissions, or related field (home health experience
preferred). - Strong
organizational skills, attention to detail, and proficiency in EHR
systems. - Excellent
written and verbal communication skills. - Proficiency
in Google Suite and other relevant software. - Ability
to multitask, prioritize, and work under strict deadlines in a fast-paced
environment. - Understanding
of Medicare, Medicaid, private insurance guidelines, HIPAA, and PHI
regulations.
