Home Health Intake Coordinator
You are a detail-oriented and compassionate
professional with experience in medical administration. You thrive in a
fast-paced environment, providing exceptional customer service and ensuring
patients receive timely and efficient home health care.
Location: Burlingame, San Jose, Fairfield, and Oakland, CA
Schedule: Temp-to-Hire, Full-Time (Potential for
Incentive Schedule; Must Be Available One Weekend per Month)
This Position Pays: $30.00–$35/hr + Monthly
Productivity Bonuses
What You Will Be Doing:
- Receive
and process new patient referrals using an industry-leading EMR system - Perform
accurate and timely data entry - Assign
new patients to Case Managers to ensure admission visits occur within 48
hours - Communicate
with healthcare providers to coordinate care - Use a
HIPAA-compliant eFax system to manage patient orders - Make
follow-up calls to patients and families to provide clear, supportive
communication - Support
daily workflow through coordination and administrative tasks
What You Bring:
- Minimum
2 years of administrative experience in a medical setting - Intermediate
excel experience required - Direct
experience with patient intake, registration, or admissions highly
preferred - Strong
organizational, communication, and multitasking skills - Familiarity
with HIPAA regulations and EMR systems - Ability
to work a consistent weekday or weekend schedule - Must be
able to work in high stress environment with a team player attitude
