Housing Case Manager

Location: San Rafael

Pay: $29.40/hr

You are a compassionate and organized professional dedicated to supporting individuals transitioning from chronic homelessness into stable housing. You excel at case management, advocacy, and collaboration with social service agencies to ensure clients receive the necessary support to thrive.

What You Will Be Doing:

• Conduct bi-monthly home visits to tenants in housing programs, assessing their needs and progress.
• Develop individualized case plans and collaborate with healthcare providers and social service agencies.
• Assist clients with independent living skills, goal setting, and accessing essential resources.
• Advocate for tenants, make referrals to appropriate services, and maintain accurate records.
• Participate in team meetings, staff training, and case management sessions.
• Support tenants in maintaining housing while encouraging self-sufficiency.

What You Bring:

• Education: Undergraduate degree from an accredited institution (Master’s in a related field may substitute for experience).
• Experience: Background in social services, case management, or working with homeless populations preferred.
• Skills: Strong organization, communication, and problem-solving abilities. Ability to maintain appropriate professional boundaries.
• Requirements: Valid CA driver’s license, satisfactory driving record, negative TB test, and COVID-19 vaccination (or exemption).
• Preferred: Bilingual abilities.
